Karen Selby, RN: What are the financial options for mesothelioma patients?
You can have the best insurance that is. There may be a co co pay, they may not be. But, travel involved, going back and forth doctor's appointments, is just the little expensive would be, but it can be just so astronomical with seeing specialists, whether if it's out of it, or if you're going out of state, it just can be just overwhelming.
So oftentimes, more times than not, a patient will say, I just can't afford this? How am how am I gonna afford this? What is this gonna cost me? There is the a benefit that there are some financial, options for them. And we do our best to explain it and help patients understand what those options are. We want them to make their own decisions on that, but our job as advocates is to educate them and guide them. So when it comes time for them to make those choices, they're making very informed decisions.
How Drug Assistance Programs Work for Mesothelioma Medications
A drug assistance program lowers or eliminates the cost of a prescribed medication for eligible patients. For mesothelioma, financial help comes from the drug’s manufacturer, independent nonprofit foundations and government programs like Medicare Extra Help. A mesothelioma regimen often includes a combination of multiple drugs, so you may end up using more than one program at a time.
Sources of Financial Help for Mesothelioma Drugs
- Government programs like Medicare Extra Help
- Manufacturer copay assistance cards
- Manufacturer patient assistance programs
- Nonprofit foundation grants
Manufacturer programs generally fall into 1 of 2 categories. A copay assistance card reduces your out-of-pocket cost if you have private insurance. Meanwhile, patient assistance programs provide the medicine at little or no cost to qualifying patients who don’t have insurance or have insufficient coverage.
Insurance coverage often determines which type of program a patient can access. Each situation is different, and the right program depends on what coverage is already in place.
“But this last time, he was doing the chemo since August of ‘23. It was a lot harder because I started to get the bills now, and that’s knocking me off my feet. I can’t believe the prices. But with good insurance, I don’t have to pay what somebody else would have to.”

Matching a Program to Your Insurance Situation
The specific programs available to a mesothelioma patient depend on their health insurance coverage. Commercially insured patients can use manufacturer copay cards, while uninsured patients leverage manufacturer patient assistance programs. Patients on Medicare or Medicaid can get assistance from independent nonprofit foundation grants and government subsidies, like Medicare Extra Help.
One critical fact that people often miss is that manufacturer copay cards exclude patients with any form of government insurance, including Medicare, Medicaid, TRICARE and VA coverage. That’s a federal legal requirement, not a manufacturer’s choice, so Medicare and Medicaid patients shouldn’t waste time applying for a copay card they can’t use.
Insurance coverage can also affect which specialists and hospitals a patient can see. As Missy Miller, Patient Advocate at The Mesothelioma Center, explains: “Insurance is a very complex thing. Some insurance doesn’t cover certain hospitals or providers … We help the patient evaluate if their insurance is actually accepted by the provider or at the hospital. And if it’s not, we guide them on what insurance plans those hospitals particularly take.”
Drug Assistance Program Options
| Your Situation | Best First Step | What It Does |
|---|---|---|
| Medicare or Medicaid | Independent foundations + Medicare Extra Help | Grants/subsidies for copays and coinsurance; copay cards aren’t an option |
| Private/commercial insurance | Manufacturer copay assistance card | Lowers out-of-pocket costs (not valid with government insurance) |
| Uninsured | Patient assistance program | Provides medicine at little or no cost to patients who meet income limits |
Manufacturer Programs for Mesothelioma Chemotherapy Drugs
Mesothelioma chemotherapy usually pairs a platinum-based drug, either cisplatin or carboplatin, with another chemo medication called pemetrexed. The assistance options differ significantly between brand-name and generic drugs, so patients often need to pursue different routes depending on which drugs they receive.
Alimta (Pemetrexed) and the Lilly Cares Foundation
Eli Lilly manufactures the popular chemo drug Alimta (pemetrexed), and its Lilly Cares Foundation provides it at no cost to qualifying patients. To be eligible, patients must meet the program’s income guidelines, be permanent U.S. residents, have no insurance (or have Medicare Part D), not receive VA benefits, not receive full Extra Help benefits and not be enrolled in Medicaid.
Generic pemetrexed is now available, so many patients receive that medication rather than brand-name Alimta. In that case, the Lilly Cares program doesn’t apply, and the options in the next section are more relevant.
Help for Generic Platinum Chemotherapy
Cisplatin and carboplatin are long-established generics with no manufacturer-assistance programs. Patients who need help covering the cost of these drugs can use charity care programs at their hospital or infusion center, or independent foundations that fund the full chemo regimen rather than a specific brand or drug. Discount tools like the NeedyMeds drug discount card can also help cut costs.
Manufacturer Programs for Mesothelioma Immunotherapy Drugs
Immunotherapy is now central to mesothelioma treatment, and these drugs are often the most expensive part of the regimen. Each major manufacturer runs 2 tracks: A copay card for commercially insured patients and a patient assistance program for uninsured patients who meet income criteria.
Keytruda (Pembrolizumab) and Merck
Merck offers 2 programs to help patients afford Keytruda (pembrolizumab). Commercially insured patients can enroll in the Merck Co-pay Assistance Program, in which patients pay the first $25 of their copay per infusion and Merck covers the rest, up to $25,000 per eligibility period. The program isn’t valid for patients with government insurance. Uninsured patients who meet income criteria can apply to the Merck Patient Assistance Program, which provides Keytruda at no cost.
Bristol Myers Squibb’s Opdivo (Nivolumab) and Yervoy (Ipilimumab)
Bristol Myers Squibb runs 2 programs for patients taking Opdivo (nivolumab) and Yervoy (ipilimumab), which doctors use together to treat pleural mesothelioma. The BMS Access Support Co-Pay Assistance Program helps commercially insured patients cover per-dose costs above the first $25, up to $25,000 per drug per year. Patients with government insurance aren’t eligible for the copay program.
The independently operated Bristol Myers Squibb Patient Assistance Foundation provides free medicine to eligible uninsured and underinsured patients who meet income requirements. Some Medicare patients are also eligible to receive assistance through this program.
Ask About Both Tracks
Every major drug manufacturer runs a copay program and a separate patient assistance program. If you don’t qualify for one because of your insurance type, ask the manufacturer’s access line which track fits your situation and whether they can connect you with an independent foundation.
Independent Foundations That Help With Mesothelioma Drug Copays
Independent nonprofit foundations offer mesothelioma grants that cover drug copays, coinsurance and deductibles. They’re the main option for Medicare and Medicaid patients, who can’t use manufacturer copay cards.
Foundations to Know
- The Assistance Fund
- CancerCare Co-Payment Assistance Foundation (includes a dedicated malignant mesothelioma fund)
- HealthWell Foundation
- TotalAssist (formed in July 2026 as the result of a merger between PAN Foundation and the Patient Advocate Foundation)
Each foundation runs disease-specific funds, which open and close based on available funding. Patients should check their current fund status and apply quickly when a relevant fund is accepting applications.
To qualify, patients generally need to be in active treatment, have insurance that covers the drug, meet an income limit based on the federal poverty level and receive treatment in the U.S. Our Patient Advocates can help identify which funds are currently open and assist with the application process.
These foundations focus specifically on drug copays. Other types of financial assistance for mesothelioma patients, including help with travel, lodging and living expenses, are available through separate programs.
Medicare Extra Help and Government Safety Nets
Medicare Extra Help, also called the Part D Low-Income Subsidy, lowers what patients pay for prescription drugs when their income and resources fall below the program’s limits. This matters for mesothelioma patients because many are 65 or older and on Medicare. Extra Help can reduce or eliminate Part D premiums, deductibles and per-drug costs, making it one of the most valuable programs available to older patients.
Some mesothelioma patients enroll in Extra Help automatically, including those who also have Medicaid, Supplemental Security Income or a Medicare Savings Program. Others apply at no cost through Social Security. Many veterans diagnosed with mesothelioma also qualify for VA benefits, which cover a wide range of medical costs through a separate system.
In 2026, Extra Help covers patients whose income falls at or below $23,940 for individuals or $32,460 for married couples, providing prescription drug discounts and covering Part D premiums and deductibles. Resource limits are $16,590 for individuals and $33,100 for married couples. The program’s estimated average annual value is $5,700 per person.
Free Tools for Mesothelioma Patients and How to Apply
Several free directories help mesothelioma patients find assistance programs for any specific drug in their regimen, and most patients don’t go through the application process alone. An oncology social worker or financial navigator at the treatment center can identify the right programs, gather documents and submit applications on a patient’s behalf. Forms typically require signatures from both the patient and the prescribing doctor.
Tools for Mesothelioma Patients
- AmericasMedicines.com (formerly the PhRMA Medicine Assistance Tool), a search engine that identifies manufacturer drug assistance programs
- NeedyMeds, a nonprofit that offers a free drug discount card and helps patients find assistance programs and affordable clinics
- RxAssist, a database of manufacturer patient assistance programs
Having a few things ready before applying makes the process faster. Patients generally need their diagnosis, drug names, insurance details and proof of income. Fund availability changes throughout the year, so checking program status early in treatment gives patients the best chance of getting help before funds close.

Get Financial HelpCommon Questions About Mesothelioma Drug Assistance
- Can I get assistance paying for my mesothelioma drugs if I’m on Medicare?
-
You can, but not through manufacturer copay cards, which exclude anyone with government insurance. Medicare patients can use independent foundations, including TotalAssist, HealthWell and CancerCare, and apply for Medicare Extra Help through Social Security. Fund availability changes, so checking the current status and applying quickly can make a difference.
- What’s the difference between a copay card and a patient assistance program?
-
A copay card reduces the amount a commercially insured patient pays per dose. A patient assistance program provides the drug at little or no cost to qualifying patients without insurance. The 2 programs serve different situations and often run side by side from the same manufacturer.
- Is there assistance for generic chemotherapy drugs like cisplatin and carboplatin?
-
Cisplatin and carboplatin are long-established generic medications with no manufacturer patient assistance programs. Patients can turn to charity care at their hospital or infusion center, independent foundations that fund the full regimen and discount tools like the NeedyMeds card.
- Do I have to be uninsured to qualify for free mesothelioma medication?
-
Patients typically can’t qualify for a manufacturer’s patient assistance program if they have commercial insurance. However, patients with insurance who still can’t afford copays may qualify for independent foundation grants. Income limits apply to both.
- How long does it take to get approved for a drug assistance program?
-
It varies. Some manufacturers and foundation programs decide within days, especially with a complete application and prescriber sign-off. Others take longer. Patients who can’t wait should ask whether an expedited review is available.
- Can my oncologist’s office apply to these programs for me?
-
Yes. Most applications need the prescriber’s signature, and oncology financial navigators and social workers routinely submit them for patients. A completed application with all required documents moves the fastest.
